Consent to the Processing of Personal Data for the Purpose of Contact
By completing this form, I hereby grant permission to the person responsible for data processing, Legal Concept LLC (Identification No.: 405739651; Legal Address: Georgia, City of Tbilisi, Vake District, Levan Mikeladze Street No. 17, First Floor, Premises No. 9; Email: info@legalconcept.ge ), to process my personal data, namely: my first name, last name, email address, phone number, and the issue stated in my message in relation to which I wish to be contacted and/or receive a consultation.
I am informed that providing personal data to the person responsible for data processing is voluntary and that the purpose of data processing is to fulfill my request, specifically to contact me in order to provide legal services and/or consultation via the phone number or email address indicated by me.
I am also informed that my personal data, as a data subject, will be processed by Legal Concept LLC for a period of 1 (one) month, except in cases where, based on my request, Legal Concept LLC provides me with paid legal services and a contractual relationship is established between us. In such case, my personal data will be processed for a period of 3 (three) years.
Furthermore, I declare that I am informed of my rights granted under the legislation of Georgia as a data subject, including my awareness that I have the right to request from the person responsible for data processing confirmation as to whether my personal data is being processed, whether such processing is lawful, and to receive the following information free of charge upon request:
- Information on the personal data being processed about me, as well as the legal basis and purpose of such processing;
- Information on the source of data collection/acquisition;
- Information on the data retention period (duration), or, if a specific period cannot be determined, the criteria used to determine such period;
- Information on the rights of the data subject provided under this Article;

I am entitled to receive the above information no later than 10 (ten) working days from the date of the request. In exceptional cases, and with proper justification, this period may be extended by no more than 10 (ten) working days, of which I must be notified immediately.
I also have the right to access my personal data held by the person responsible for data processing and to receive copies of such data free of charge, except in cases where a reasonable fee has been established by the data controller due to the resources expended in providing the data in a format different from the storage format and/or due to the frequency of requests.
I am entitled to access such data and/or receive copies thereof no later than 10 (ten) working days from the date of the request. In exceptional cases, and with proper justification, this period may be extended by no more than 10 (ten) working days, of which I must be notified immediately.
I also have the right to request the person responsible for data processing to correct, update, and/or supplement inaccurate, incorrect, and/or incomplete data concerning me. Such data must be corrected, updated, and/or supplemented no later than 10 (ten) working days from the submission of the request, or I must be informed of the grounds for refusal and the procedure for appealing such refusal.
If the person responsible for data processing independently identifies that the data held is inaccurate, incorrect, and/or incomplete, such data shall be corrected, updated, and/or supplemented within a reasonable period, and I shall be notified within 10 (ten) working days from the date of correction.
I have the right to request the person responsible for data processing to suspend the processing of my personal data, or to delete or destroy such data. No later than 10 (ten) working days from the submission of such request, the processing shall be suspended and/or the data shall be deleted or destroyed, or I shall be informed of the grounds for refusal and the procedure for appealing such refusal.
The person responsible for data processing has the right to refuse to satisfy such a request if:
- a) there exists a legal basis for data processing as provided by law;
- b) the data is processed for the purpose of establishing, exercising, or defending a legal claim;
- c) the processing is necessary for exercising the right to freedom of expression or information;
- d) the data is processed for archiving purposes in the public interest, scientific or historical research, or statistical purposes, and the exercise of the right to suspend, delete, or destroy the data would render the achievement of processing purposes impossible or significantly impair them.
I have the right to receive information on the suspension, deletion, or destruction of data immediately upon the execution of the relevant action, but no later than 10 working days.
